Getting to Know You Questionnaire

I'm really excited to get to know each and every one of my students!  This is an in class assignment for the first day of school.  As part of the introduction process you will need to fill out a questionnaire about yourself.  Be as honest as possible with your responses.  Answer the following questions on a sheet of paper and drop it off in the Turn In Box in Room #153.  Don't worry! Your answers will not be shared with the rest of the class.  Your responses will be private!  

  1. What is your Photo class period?

  2. What is your full name? (as seen on the roll sheet)

  3. What pronouns would you prefer?

  4. What name would you prefer to be called?

  5. What grade are you in?

  6. Whom do you live with? (for example: Mom & 2 brothers or I travel between 2 homes.  If you are split between multiple homes please note that.)

  7. What pets do you have and what are their names?

  8. If you could have one super power what would it be?

  9. What are your pet peeves?

  10. What was your best memory of the summer?

  11. What are you currently excited about or looking forward to?

  12. What is your idea of a good teacher?

  13. What is your idea of a bad teacher?

  14. What are you currently anxious and/or nervous about?

  15. If you could go anywhere int he world, where would you go and why?

  16. If you could have one wish, and it wasn't for wealth or wishes, what would it be?

  17. What was the hardest thing about returning to school full time after Covid and distance learning?

  18. What or who do you want to be? Now? In the future?

  19. What is something surprising about you?

  20. What are you passionate about? 

  21. Most importantly, what should I know about you? What do you want me to know about you that will help me be a better teacher to you? (The answer for this can really vary, but we all have something! It could be about how you learn best, fears you have, medical info, mental or emotional info, maybe you work a job, maybe you are color blind, maybe you just moved to town, or your folks are in the middle of a divorce, or maybe you are super excited and passionate about photo and there is something specifically you'd like to learn!)
